The practice is registered and complies with the Data Protection Act 1998. Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act. Please contact the Practice Manager for further information.
We have an open list and welcome requests for registration from patients living in or moving to the practice area.
You can register by completing the relevant forms which are available from our reception, this also give you chance to pop in and get to know our surgery.
Please ensure that you complete all of the sections on the forms including your NHS number and full details of your previous doctor. This will enable us to retrieve your medical notes more quickly.
These registration forms will need to be completed for each member of your family that wishes to register with our practice.
You will need to provide proof of identification and address when handing in your registration forms. You will need to provide one of the following documents from each of the lists:
Proof of Identity
Passport / Driving Licence / Birth Certificate / Marriage Certificate / Medical Card
You will be registered under one of our doctors but you are free to see any doctor for any problem.
We aim to treat our patients courteously at all times and expect our patients to treat our staff in a similarly respectful way. It is your responsibility to keep your appointments, inform us of your past illnesses, medication, hospital admissions and any other relevant details.
If you change name, address or telephone number please log into your online services account to update your details, alternatively you can visit our reception to complete a change of details form. If you move outside the practice area you may need to find a doctor in your new area.

There is access for the disabled to all of our consultation rooms. Two designated parking bays are adjacent to the surgery entrance and the surgery has a ramp and wide automatic doors to allow for wheelchair access.

We take seriously any threatening, abusive or violent behaviour against any of our staff or patients. If a patient is violent or abusive, we will exercise our right to take action to have them removed, immediately if necessary, from our list of patients.
We are a training practice and you may be offered an appointment with a GP registrar. These doctors are fully qualified and will have had experience in several hospital specialties. They are completing their specialist training as a GP.
We also teach medical students and sometimes you may be asked to see the doctor or nurse with a student present. You will always be asked beforehand if this is acceptable and you may ask for the student to withdraw if you prefer to have your consultation in private.
An increasing part of all undergraduate and postgraduate training is the use of video consultations. You will be advised when booking an appointment if the doctor will be videoing the consultation. Written consent is needed prior to seeing the doctor. The recording is used for teaching purposes only and can be deleted during or immediately after the consultation if you wish.
